I am not an expert on types of arrowheads and blades ,but I believe this to be a Hopewell or Archaic Cobb Knife Blade.  Measures 5 and 1/4th inches long.The indent on the blade is not damage.You can see with magnafication that it has been chipped or flaked in during sharpening for whatever reason.Material looks to be from flint ridge.
